what I usually do is I hold the pipecleaner in the same hand as my yarn and I work the yarn around the pipecleaner as I crochet,I use single stitches so it covers the whole pipe cleaner then I make the leaves and use a thread and needle to sew the leaves to the stem.

I have crocheted many flowers and leaves and I've usually used the "16 or #18 covered stems. You can get packages of these at any craft store. I usually make my flower first, then I fold the wire in 1/2 then poke both ends down through the center of the flower. The bent part of the stem is usually not visible. Then I use florist tape and start twisting downwards in a spiral until I get to the end. You can do the same thing with the leaves and you could use a shorter, thinner wire and place them up against the sth stem in the position you prefer, and twist the floral tape around the stem wire and that will hold it in place. You can use pipe cleaners the same way but they would not be as rigid.
